For thousands of people living with kidney failure, dialysis is not just a treatment; it is what keeps them alive. While most patients are familiar with the dialysis machine and the hours spent connected to it every week, many are unaware that the treatment depends on something much smaller: a specially created blood vessel in the arm called an arteriovenous (AV) fistula.
An AV fistula is considered the best and safest way to access the bloodstream for haemodialysis. It allows blood to flow efficiently to and from the dialysis machine, making treatment more effective while reducing the risk of infections compared with temporary catheters. However, despite its importance, fistula care often receives far less attention than it deserves.
Doctors warn that many patients lose their fistulas simply because they fail to recognise early warning signs or do not know how to protect them. The loss of a fistula can interrupt dialysis, increase the risk of infections, require additional surgeries and add emotional and financial stress to patients who are already managing chronic kidney disease.
According to Dr Avik Bhattacharyya, Senior Consultant – Interventional Radiology, CK Birla Hospitals, CMRI, better awareness and early action can prevent many of these complications.
WHY AN AV FISTULA IS SO IMPORTANT
An AV fistula is created by surgically joining an artery and a vein, usually in the arm. This increases blood flow through the vein, making it stronger and larger so that it can withstand repeated needle insertions during dialysis.
Without a healthy fistula, haemodialysis cannot be performed effectively. That is why nephrologists often describe it as a patient's "lifeline."
Unlike temporary dialysis catheters, AV fistulas generally last longer, have a lower risk of infection and provide better blood flow for dialysis. However, they need regular monitoring and proper care to remain functional.
INDIA'S GROWING DIALYSIS BURDEN
The need for dialysis continues to rise across India.
Every year, nearly 2.2 lakh people develop end-stage kidney disease, creating a demand for more than 3.4 crore dialysis sessions annually.
West Bengal alone has 74 government-supported dialysis centres under the Pradhan Mantri National Dialysis Programme, in addition to several private dialysis facilities. The state records around 2,000 dialysis sessions every day, highlighting how many lives depend on reliable dialysis access.
Behind nearly every one of these dialysis sessions is an AV fistula.
WHY PATIENTS LOSE THEIR FISTULAS
One of the biggest challenges doctors see is that patients often seek medical help only after the fistula has already developed serious problems.
Many ignore the early warning signs because they assume they are a normal part of dialysis.
By the time they consult a vascular specialist or interventional radiologist, the fistula may have already developed significant narrowing (stenosis) or become completely blocked.
According to Dr. Bhattacharyya, many of these complications could be prevented with earlier diagnosis and treatment.
In clinical practice, nearly 30% of patients eventually lose their fistula because they report problems too late, even though timely treatment might have saved it.
WARNING SIGNS YOU SHOULD NEVER IGNORE
Doctors advise patients to examine their fistula every day.
A healthy fistula produces a continuous buzzing or vibrating sensation, known as a "thrill." If this vibration becomes weaker or disappears, it should be treated as a medical emergency.
Other warning signs include:
- Swelling in the fistula arm
- Redness around the access site
- Pain near the fistula
- Bleeding that continues longer than usual after dialysis
- Difficulty inserting dialysis needles
- Reduced effectiveness of dialysis sessions
These symptoms are not routine side effects. They may indicate that blood flow through the fistula has become restricted.
FIVE SIMPLE HABITS THAT HELP PROTECT AN AV FISTULA
Doctors say a few everyday precautions can significantly increase the lifespan of a fistula.
1. Check the vibration every day
Feel the fistula daily to make sure the characteristic buzzing sensation is still present.
2. Protect the fistula arm
Never allow blood pressure measurements, blood tests or intravenous injections in the arm with the fistula, as these can damage the blood vessels.
3. Avoid unnecessary pressure
Do not carry heavy objects with the fistula arm and avoid sleeping on that side, as prolonged pressure can affect blood flow.
4. Keep the area clean
Good hygiene reduces the risk of infection around the fistula site.
5. Report changes immediately
If you notice swelling, prolonged bleeding, reduced vibration or any change in blood flow, contact your healthcare team without waiting for your next dialysis appointment.
EARLY TREATMENT CAN SAVE THE FISTULA
The good news is that not every fistula problem requires surgery.
If narrowing is detected early, doctors can often perform a minimally invasive procedure called fistuloplasty (balloon angioplasty). During this image-guided procedure, a small balloon is used to widen the narrowed blood vessel and restore normal blood flow.
This can preserve the existing fistula and allow patients to continue dialysis without major interruption.
However, timing is critical.
A fistula treated early can often be saved. Waiting several weeks may make it impossible to restore.
WHY PATIENT AWARENESS MATTERS
Doctors say there is still limited awareness about fistula care among dialysis patients.
While patients usually receive detailed instructions about medications and dialysis schedules, they are often given little information about examining their fistula or recognising early warning signs.
Experts believe every dialysis centre should include fistula education as a routine part of patient care.
